First Premium^® Insurance Group, Inc., CMGA values its producer relationships and is open to new producers who share our professional and ethical values. We are currently accepting applications from producers licensed by one or more of the following states: Louisiana, Mississippi (surplus lines required), Florida or Alabama.

All interested prospects should complete the producer application and attach a copy of their E & O declaration page and Mississippi Surplus Lines License (if applicable). Upon review, additional documentation and forms may be required.

If you are approved, we will send the First Premium^® producer agreement and blank W-9, to be signed and returned with copies of the agency and agents' licenses. Once we have received all of the required documents, we will email a welcome letter with your approved lines of business as well as your user name and password, so you can quote and bind immediately. Your agency will have exclusive access to our forms, guidelines and on-line rating system.
